Identification
- Photo ID for all household members 18+ yrs old (e.g. a driver’s license or school ID)
District Residency & Rent Owed
- Current lease agreement. If you do not have a copy of your lease, work with your provider to identify other appropriate document(s).
- Documentation of back rent owed from your landlord/co-op Board (e.g. a ledger from your landlord showing rent missed by date and balance due, and payment deadline)
COVID-19 Related Eligibility
- Supporting Documentation of COVID-19 Related Impact, if available (e.g. termination of employment or notice of daycare closure)
Income Eligibility
- Proof of income for all household members 18+ yrs old
  - Signed 1040 Tax Return for 2020 OR
  - Income from the last TWO (2) Months (e.g. Checking and savings account statements, Unemployment or emergency benefit statements, or Transaction histories from CashApp, Venmo, PayPal etc.)
